OPPORTUNITIES to PARTICIPATE

The purpose of this survey is to gain insight into the needs and demographics of the UAY population in the Tampa/Hillsborough County CoC in a brief format that can be useful in efforts to create a system of services tailored to the specific needs of this population.

Please note that the survey will be anonymous, unless the Youth chooses to provide contact information to receive follow up assistance and/or information about the Youth Action Board.
